To help your doctor prepare for your health review, we've put together a set of practical information sheets for you to complete.

Our aim is to help you understand the examinations we offer.
Prepare for your consultation and understand :
- Consult the memo sheet "I prepare my consultation in 10 steps"
- Fill in the 6-page "I prepare my consultation" self-questionnaire
- Understand "My blood test "and "My kidney test"
- Make my blood pressure self-measurement 3 days before the consultation and present the results to your doctor
Prepare my medical follow-up binder:
- Fiche mémo " Preparer mon classeur médical de suivi"
- Fiche "La santé de mon artère" to be filled in before the consultation and presented to my doctor.
